If you would like to arrive for your first treatment as prepared as possible, please click on the buttons to the right. Download and print the New Patient Intake Form and Informed Consent Form.

Please sign the New Patient Intake Form and Informed Consent Form and bring them both with you to your first appointment. Alternately, you may arrive at your appointment 15-minutes early and fill-out the intake and consent form then.

Follow-up treatments will not require any more forms or signatures.
